When we look at a painting our brains scan it for information. The artist controls this information. By using design and composition we are able to capture the attention of the viewer, getting them to spend time looking at the painting, hopefully without getting bored. Ideally the composition will lead the viewers eye around the painting, taking everything in but ultimately coming back to rest on the "focal point", the most important thing, the reason for the painting.
Composition is the arrangement of line, tone and colour in a way that makes sense and is aesthetically pleasing. A key element in the success of a painting is a good composition.
